The Basics of Eyelid Surgery

Eyelid surgery, also called a blepharoplasty, is a procedure that removes extra skin and fat around the eyelids to restore a more youthful appearance. A procedure can also be done if your eyelid droops too low over your pupil. This is called ptosis repair. These surgeries can have dramatic results with scars that are often barely visible!

Upper Eyelid Blepharoplasty

This procedure is done for too much skin or bulging fat in the area of the upper eyelids. An incision is made in the natural crease in the eyelid. Extra skin and fat are removed through this incision. This incision heals to become a very well hidden scar.

Ptosis Repair

This procedure is done for eyelids that are drooping too low over your pupil. Normally, the eyelid is kept in the right position by the muscle that raises your eyelid called the “levator.” This muscle is connected to the hard structure in your eyelid near the eyelashes called the “tarsus.” A drooping eyelid is usually caused by the stretching, thinning, and sometimes breakage of this connection. A ptosis repair uses the same incision as the blepharoplasty to restore the levator’s proper connection to the tarsus.

Lower Lid Blepharoplasty

This surgery removes extra skin and fat from the lower eyelids for a smoother contour. Depending on what you need, an incision may be made inside your eyelid and/or outside the eyelid in a natural skin crease. Sometimes fat needs to be removed. Other times it needs to be repositioned. And sometimes, the eyelid itself needs to be tightened. Many women may be good candidates for eyelid surgery. Many times the issues we see with our eyelids get worse with age. Eyelid surgery may be right for you if you feel that:

• your eyelids seem heavy and give you a tired appearance

• your eyes seem too hollow

• your lower eyelids have folds of skin or fat bulges underneath

There are a few preparations that are important before eyelid surgery. It is helpful to see an ophthalmologist beforehand to make sure your eyes are healthy. You want to get any issues such as dry eyes under control before proceeding with surgery. Make sure you are eating a balanced diet, get enough protein and plenty of rest. This helps with healing. Do not smoke and avoid being around people who are smoking.

During the initial consultation, you will be asked about your medical history to ensure you are in good health and a candidate for blepharoplasty. You will then discuss eyelid surgery specifically and your cosmetic goals.

Dr. Mason will examine you so you can decide together on the right surgery for your body. Remember, there is no one-size-fits-all. Dr. Mason will customize a plan just for you.

Expect a mixture of anxiety and excitement. Both are normal. Remember, you’re doing something for yourself, and you should feel excited! You will have a scheduled arrival time on the day of surgery.

The Surgery Center team will be expecting you and check you in upon arrival. Please do not eat the morning of surgery. Dr. Mason will come to talk to you before surgery and make marks on your skin where your incisions will be; these will be discussed in your treatment plan. You will be able to talk about this, ask questions, and agree on it together. She will make sure there have been no changes to the original plan you agreed to. You will also meet your anesthesiologist.

From there, you will go back to the operating room, and the anesthesiologist will put you to sleep. This is where you say goodbye to your tired eyelids and await your incredible results. The surgery can take anywhere from 2 to 4 hours, depending on your surgery plan. You will be brought to the recovery area, and once you are released, you will be ready and able to go home the same day.

While you are taking narcotics pain medication, you will not drive, so you will need to arrange for someone to pick you up from the surgery center. This is a good time to switch your mindset to “let them look after me for a while,” as you will need your rest.

You will be quite bruised and swollen for 1-2 weeks after surgery. During this time, you may need to use eye drops to keep your eyes moist. Although blepharoplasty is not a particularly painful procedure, the swelling and bruising can be significant, and you may wish to take some time off work. This is the time for you to relax, heal, and take care of yourself.
